A 2004 study in Biological Psychiatry found modafinil improved cognition and response inhibition in adults with ADHD. A 2006 Pediatrics study reported reduced impulsivity in children and adolescents. Sounds promising.
But here’s the twist. Later, larger trials failed to beat placebo on standardized ADHD rating scales. And safety concerns about skin reactions in kids led regulators to decline ADHD approval. That’s why modafinil stays strictly off-label for ADHD, and why any use needs careful medical oversight.

These numbers come from modafinil’s FDA-approved uses (narcolepsy, sleep apnea, shift work disorder). Any ADHD use is off-label. Your prescriber sets the dose based on your situation.
Adult Dosing
Standard dose for approved indications: 200mg once daily in the morning.

What Affects Your Ideal Dose
Body weight and metabolism. Bigger people or those with faster liver metabolism may burn through modafinil quicker. Smaller individuals often do fine on 100mg.
Other meds. Some drugs change how modafinil is metabolized: antifungals like ketoconazole, certain antibiotics, and hormonal contraceptives. Modafinil can reduce the effectiveness of the pill. Tell your prescriber everything you’re taking.
Individual variation. Some people feel 100mg strongly. Others need 200mg to notice anything. Start low, work up. That’s the safest path.
Timing and Administration
Modafinil’s half-life is roughly 12-15 hours (including active metabolites). That’s a long time. Get the timing wrong and you’ll be staring at the ceiling at 2am.
When to Take It
For narcolepsy or sleep apnea: Once daily in the morning, within an hour of waking.
For shift work: About an hour before your shift starts.
General rule: Take it before noon. Anything after early afternoon significantly increases insomnia risk. Individual tolerance varies, but this is a solid baseline.

Split Dosing

Food: You can take modafinil with or without food. A light meal can reduce nausea and slightly delay peak effects, but doesn’t change overall absorption much.
Side Effects and Safety
Compared to amphetamines, modafinil is generally easier on the body. But side effects happen, especially when you’re starting out or pushing the dose too high. If they’re severe, stop and talk to your prescriber. Don’t try to adjust on your own.
Common Side Effects
Headache. Most common one. Usually linked to dehydration. Drink 2-3 liters of water daily and take it with food. That fixes it for most people.
Reduced appetite. Not as bad as amphetamines in my experience, but it’s there. Eat regular meals even if you’re not hungry.
Insomnia. Almost always caused by taking it too late in the day. Stick to morning dosing and this mostly disappears.

Serious Side Effects: Stop and Get Help
Seek Immediate Medical Attention For:
- Skin rash, particularly if spreading, blistering, or accompanied by fever
- Facial swelling, swelling of lips, tongue, or throat
- Difficulty breathing or swallowing
- Hives or severe itching
- Significant mood changes, including depression, agitation, or thoughts of self-harm
- Chest pain or irregular heartbeat
